Child Record Form 
 
This is a great form to help you gather the information you need about your mindees in accordance with the EYFS Statutory Framework (3.73) and the Child Protection Act 1989.  It's best to ask parents/carers to fill it out and return it prior to their child starting in your setting, so that you have all the necessary information.
 
This form provides space to assertain the following:
oersonal information about the child,
additional information about the child, including their personal needs such as medical, special education or dietary
contact information about parents/carers
identifying those with parental responsbility
nomiated authorised persons/emergency contacts
