Parent/Carer Feedback
This 2 page feedback form is simple for parents/carers to fill out and will help you gather the honest opinions of the families you work with in atactful way. Having a strong parent/carer partnership is a huge part of the new EYFS Framework (2.3 and 3.74) and this is positive way to to acheive that.
The questions are straightforward and cover different parts or your business, such as their opinion on the way you take care of their child on a day to day basis, how they think you've handled the covid situation within your setting and asking for their opinions on the meals you provide.
If not all the questions included are pertinent to your setting, you can simply make your own changes using the editable word document and replace the standard questions with your own. If you're happy with them, you can print the PDF version and hand them out to your families.
If you prefer, you could email it to them, but make sure you can send over the Word document so that parents/carers can type in their answers instead of printing and handwriting. This may be a great option if English is not the first language for your families, as they could use something like Google Translate to share their thoughts with you.

- This digital pack is a Zip File containing both PDFs and editable Word documents.
